Mario World 2: Yoshis Island is a two-dimensional, side-scrolling platform game, similar to the early Super Mario titles. This game tells the story of baby Mario and baby Luigi. A stork was delivering them to their parents, but Kamek, an evil character, attacked the stork and took baby Luigi. Baby Mario fell into the sea and landed on Yoshi's Island, where Yoshi, a friendly dinosaur, found him. Baby Mario had a map that showed the location of Bowser's Castle, where baby Luigi was taken. Kamek realized he missed baby Mario and sent his followers to capture him. The Yoshis decided to help baby Mario and formed a relay team to carry him through the island. They faced challenges and fought bosses in different forts and castles. Eventually, they defeated Kamek and rescued baby Luigi and the stork. The babies were safely delivered to their parents, thanks to the Yoshis' efforts.
In Mario World 2: Yoshis Island, there is a time-based health mechanic. When Yoshi gets hit by an enemy, baby Mario gets separated from him and floats in a bubble. A Countdown Timer appears, and Yoshi must retrieve baby Mario before the timer reaches zero. If the timer runs out, Kamek's Toadies take baby Mario, and the player loses a life. The timer starts at ten seconds and can be increased by collecting Stars, using Point Stars, and entering Middle Rings. If the timer falls below ten, it slowly increases back to ten after rescuing baby Mario. Entering a Middle Ring also serves as a checkpoint in case Yoshi loses a life.
The game consists of six worlds, each with nine levels. The goal of each level is to reach the Goal Ring roulette. By collecting five Special Flowers in a level, the player can make five spots on the roulette winning spots. Landing on a flower point allows the player to play a Bonus Challenge, where they can earn lives or special items. Two levels in each world are forts or castles where the player must defeat a boss instead of reaching a Goal Ring. Bosses are usually regular enemies who have been enlarged by Kamek.
Throughout each level, there are collectibles to find, including Stars, red coins, and Special Flowers. At the end of each level, the player's score is tallied based on these collectibles, with flowers worth ten points each and red coins and Stars worth one point each. Getting a perfect score of 100 in all levels in a world unlocks a Bonus Challenge and an extra level with its own unique challenges. If the player achieves a perfect score in the extra level, a Star is added to the game's title screen, with a maximum of six Stars.

In the game, you control Yoshi, the friendly dinosaur, who can run, jump, and do much more. One unique ability Yoshi has is the power to ingest enemies and manipulate objects using his long tongue. When Yoshi eats an enemy, he can either spit it out or swallow it. If he keeps an enemy in his mouth for a while, he will eventually swallow it. Swallowing certain enemies allows Yoshi to lay Yoshi Eggs, which he can carry up to six of. However, if Yoshi eats another enemy while carrying eggs, the first egg in line will be discarded.
Yoshi can use these eggs as projectiles to damage enemies, activate objects, bounce them off walls, or skim them across water. The player can aim the eggs using a constantly moving target reticle. While aiming, Yoshi can still move around and jump. The eggs come in different colors: green, yellow, and red. Green eggs can bounce three times, yellow eggs bounce twice, and red eggs bounce only once. Additionally, Yoshi can find power-ups in the form of metamorphosis bubbles, which allow him to transform into different forms, and the Super Star, which turns baby Mario into Powerful Mario. Yoshi also has a move called Hovering Jumps, which allows him to jump multiple times in the air, increasing the distance he travels.
