Dr. Mario is a puzzle game where you play as Mario, who is a doctor. The goal is to destroy viruses on the screen by using colored capsules that Mario throws into a medicine bottle. You need to line up capsules of the same color to eliminate the viruses. As you progress through the game, you have to clear all the viruses in each level.

The gameplay is similar to Tetris. The capsules fall vertically, and you can move them left or right and rotate them. When four or more matching colors of capsules and viruses touch, they disappear. Any remaining capsules that are not supported will fall to the bottom, and new matches will also disappear. The main objective is to remove all the viruses from the playing field to complete each level. If the capsules fill up the bottle's narrow neck and obstruct it, the game is over.
